ALTA LOMA HOSPICE

We are a team of experienced health care professionals who are proud to work alongside experts who help facilitate the delivery of our compassionate care. Delivering leading-edge patient care is no easy task, but having the right team makes that vision possible every day at Alta Loma Hospice, Inc. Here are brief overviews of some of the services you can expect to have available to you and your family 24 hours a day and 7 days a week all at no costs to you with medicare eligibility.

HOME HEALTH AIDES

Our Certified Home Health Aides (CHHAs) deliver compassionate care through assistance with daily cares, personal hygiene, assisting with feeding, and just about all activities of daily living with specialized training to do so in within a medical setting or at home.

SOCIAL WORKERS

The hospice care experience can be described by some as a journey. Our Medical Social Workers (MSWs) are there to offer individuals and their families the tools and support necessary to assist them with the journey.

SPIRITUAL SERVICES

We understand that caring for individuals and their families are all encompassing. Care is just as much spiritual as it is physical. Our non-denominational chaplain services are available to families to offer prayer and counseling. We can also work with an individual's existing religious support system. 

NURSES

Our Licensed Vocational Nurses (LVNs) and Registered Nurses (RNs) offer expertise in hospice care and perform health assessments, pain and symptom management, wound care, as well as developing nursing care plans for ongoing monitoring of patient and family needs. 

PHARMACY

Our contracted pharmacy is available 24/7 with expert pharmacists ready to answer any questions regarding medications/prescriptions as well as offering stat delivery direct to facilities or to home with nearly all our patients paying nothing out of pocket.

VOLUNTEERS

Our organization works with a variety of volunteers who help provide care and offer companionship through a variety of ways including arts, music therapy, pet therapy, activities and some through active listening and engagement. All volunteers undergo training prior to engaging our patients in a healthcare setting.

PHYSICIANS

Our in-house medical doctors provide expert medical management of terminal diagnoses as well as specialized pain and symptom management in the hospice setting. They are available around the clock for quick responses to any changes in condition or new onset symptoms.

MEDICAL EQUIPMENT

Delivery of care requires not only having the right team in place, but it also requires having the right tools available. With our contracted Durable Medical Equipment (DME) provider you can rest assured we have the right tools available and delivered right to your home.

COUNSELING

The journey through hospice care is not easy, we help individuals and their families and offer counseling every step of the way. Through our resources, our optional group counseling sessions, and our bereavement process. We are here to walk with you every step of the way and will continue to follow up. We deeply value the relationship we build with families throughout this process.
